The form must be correctly filled to avoid complications in the process of changing your registered agent. Note that there’s a fee of 15 dollars that must be paid for the document to be processed. The 15 dollars payment can be made through money order, a check, or credit card. The check must be made payable to the “Utah Secretary of State”. You can choose to complete the process online, or submit the papers and check through the mail, fax, or in person.
